Dr.
Diehl will review your forms and our office
will contact you within five business days
of receipt in order to help you determine
the best plan of action for your lifestyle.
Appointments will only be scheduled AFTER
the intake forms have been received and
reviewed.
Dr. Diehl is an out-of-network
provider to all insurance companies,
including Medicare and Medicaid.
We will provide you with a receipt
for each of your phone and office
consultations, which you can then file on
your insurance.
Please note that we can
provide necessary orders for lab testing,
whether blood, urine, saliva, or other, only
after having seen you for ONE physical
office consultation.
The First Visit:
The
Initial In-Office Consultation includes an
uninterrupted complete hour face-to-face
with the doctor.
In order for the doctor to order
lab testing for you and to treat you with
medications or supplements, the legal
standard of care requires that you have
One physical office visit with us.
If you desire, we also offer
complete physical examination at the initial
or subsequent office visits, including Pap
smear and full gynecologic examination.
Appointment and
Cancellation Policies:
We pride ourselves on
delivering personalized care focused
directly on you, and therefore, we do not
double-book appointments.
Thank you for helping us to
maintain this individualized approach to
your health by reserving your appointments
with a credit card.
A pre-approved credit card is
required in order to book all appointments,
and we keep your information strictly
confidential.
Our initial In-Office
Consultation costs $350.00.
A $25 non-refundable administrative fee is charged to your credit
card at the time of scheduling your
appointment.
This fee is non-refundable and is
not credited towards your phone or office
consultation.
We require
2
business days notification in order to
cancel an appointment and avoid being
charged for the appointment.
If you are unable to
keep your appointment as scheduled, please
let us know 48-hours in advance in order to
avoid a missed appointment charge. To
cancel an appointment, please call
864-834-6652 and leave a voice message if
our office staff is unavailable to answer
the phone.
Method of Payment:
We accept Visa, Mastercard, Discover, and checks. We do not
accept American Express at this time.
Payment is due at the time of each
appointment, and in the case of phone
consultation, we bill your credit card just
prior to the consultation.
Phone Consultations:
While a physical visit
to the office will provide you with the most
desirable standard of diagnostic medical
care, we realize that such an appointment
may not be realistic for you at this time.
Therefore, we
offer Phone Consultations both for new patients as well as for
follow-up visits.
Phone consultations might
serve you well prior to establishing an
In-Office Consultation, and may also be done
regularly after one In-Office Consultation.
Phone Consultations cost
$350.00 for the hour, and subsequent phone
consultations are pro-rated per 15 minutes.
Executive
Profile:
Many of our patients wish to perform
comprehensive testing at the initial office
visit. Therefore we offer the EXECUTIVE
PROFILE for $1500.00
This consultation includes the
following:
One and one half hours with Dr. Diehl.
Complete physical examination excluding pap
smear
Salivary kit for adrenocortical function:
this method involves four samples of saliva
to measure
4 cortisols and total DHEA.
Salivary kit to measure the reproductive
hormones estradiol, progesterone, and
testosterone.
12 hour urinary test kit to measure both
nutrient and toxic minerals/metals
A lab requisition to the lab your insurance
requires you to go to for blood testing:
Simply verify with your insurance which lab
is "in-network", what your deductible is,
and what
percentage of blood work is covered if
ordered by an "out-of-network" physician
(Dr. Diehl)
We will provide the requisition for
appropriate blood testing, including
comprehensive thyroid and
whatever else is indicated by the history
and physical examination. The lab you direct
us to
send you to will bill your insurance
directly, and your responsibility for the
bill depends on
information provided to you by your
insurance company.
Dr. Diehl is not involved
in the billing
regarding your bloodwork.
If insurance doesn't cover blood work, or for
any other reason, Dr. Diehl will direct you
to the
appropriate independent lab for bloodwork,
and you will pay that facility directly.
There are
several excellent choices for this service,
including www.mymedlab.com. and the
Hendersonville, NC
American Red Cross has the best value by
far.
You may also utilize your Health Savings
Account to cover office visit and testing
fees.
You will want to do one follow-up
appointment to go over the results of the
above testing in person.
The follow-up visit will last about an hour,
and will cost $750.00.
Subsequent visits can be accomplished by
Phone Consultation.
Elite
Profile:
For those of our patients who just need a
quick check-up we offer the ELITE
PROFILE for $350.00. This consultation
provides you with an uninterrupted 1/2 hour
with Dr. Diehl as well as a Saliva kit to
measure the reproductive/sex hormones:
estradiol, progesterone and testosterone
